The Office of Diversion Control of the Drug Enforcement Administration has just posted on the Internet several reporting forms and applications required by the Controlled Substances Act.

Of particular interest to practitioners are DEA form 106, "Report of Theft or Loss of Controlled Substance" and form 41, "Registrants Inventory of Drugs Surrendered," the form used when destroying outdated controlled substances.

The forms are online at the Diversion Control Program Web site, www.DEAdiversion.usdoj.gov.
