SUBMISSION INSTRUCTIONS:
- Prepare a total of five hard copies of your report
- Mail a hard copy of your report directly to each site team member from your last site visit. AVMA staff will provide you with their mailing labels.
- Mail one hard copy to the AVMA office.
- Email a copy to the AVMA at jhorvath@avma.org. If appendicies are not available electronically, just email the narrative of the report.
